Position: Nurse Practitioner – Youth Wellness Hubs Ontario (YWHO)
Location: Bancroft, with some travel in Hastings & Prince Edward counties
Status: Regular Full-Time - 35 hours/week, with a varied work schedule based on activities of the youth/need for services. Some evening and occasional weekend work.
Salary: $110,328 to $135,266
Description
Children’s Mental Health Services (CMHS) is a publicly funded not-for-profit agency with office locations throughout Hastings and Prince Edward Counties. As a Lead Agency for Hastings, Prince Edward, and Northumberland (HPEN) we oversee and coordinate the delivery of mental health services and programs in their region and work with community partners to provide a wide range of supports for children and youth. We work collaboratively with other child and youth mental health service providers and other affiliated community partners including our child welfare, health, and school board partners, and parents and youth engaged in our services.
Bancroft Youth Wellness Hub is part of the Youth Wellness Hubs Ontario (YWHO) provincial initiative. YWHO is an Integrated Youth Services (IYS) initiative designed to address the service gaps in the youth mental health and substance use sectors in Ontario.
YWHO supports local service providers to work together to provide young people in Ontario aged 12 to 25 with low barrier access to a full range of services including mental health and substance use supports, primary care, education, employment, housing and other social services all through youth-friendly access points. YWHO services are designed to improve experiences and outcomes for youth by:
Increasing access to rapid, low-barrier services;
Reducing transitions by providing care in one location; and,
Providing tailored, high-quality programs co-developed with youth to meet their needs.

Position Summary
Reporting to the Clinical Services Director, the Nurse Practitioner will serve as an integral member of a diverse interdisciplinary team, supporting the overall health and wellness of youth, including those facing mental health and substance use challenges. Bringing a strong primary care perspective, the Nurse Practitioner provides clinical leadership and advances excellence in care from a primary care lens in all areas of Advanced Practice Nursing.
The Nurse Practitioner will provide episodic primary care to children and youth, with a focus on holistic, evidence-informed care. Key responsibilities include clinical practice, education, mentorship, leadership, and involvement in quality improvement initiatives. The role is both population- and scope-based, emphasizing collaboration with community partners in the mental health field and medical specialists to ensure comprehensive care.
The ideal candidate brings strong primary care experience, clinical judgment, and a commitment to continuous learning. You are an effective communicator, organized, and skilled in working within diverse, interprofessional teams. Flexibility, accountability, and a focus on equity and quality are essential. Experience supporting the unique needs of youth and their families in a fast-paced setting is a must.
Main Duties and Responsibilities
Provide comprehensive, episodic primary care to children and youth, including assessments, diagnoses, treatment planning, and follow-up care.
Conduct thorough health assessments, including history-taking and gathering collateral information from caregivers, community partners, and others (with youth consent).
Collaborate with inter-professional teams to deliver and coordinate care, including prescribing medications, ordering tests, and making referrals as needed.
Emphasize health promotion, education, disease prevention, and use of psychotherapeutic approaches in clinical visits.
Support measurement-based care using standardized tools to monitor progress and outcomes.
Lead or participate in complex care planning and adapt care plans based on ongoing evaluation of youth needs.
Practice trauma-informed, anti-oppressive, and culturally safe care, aligned with health equity, harm reduction, and inclusion principles (e.g., 2SLGBTQ+).
Demonstrate strong communication skills, including empathy, active listening, and culturally respectful interactions.
Provide services in person, virtually, or remotely, based on youth needs and preferences.
Advocate for safe, equitable care and lead practice improvements aligned with the YWHO Model.
Other related duties as assigned.
Experience/Skills and other Requirements
Masters of Nursing (or equivalent with Nurse Practitioner certification)
Registered in the Extended Class with the College of Nurses of Ontario, including prescribing of controlled substances
Willingness to pursue subspecialty education in mental health, addictions, and gender-affirming care
Proven ability to assess, diagnose, and manage episodic primary care for youth aged 12–25
Proficient in prescribing, ordering tests, and providing therapeutic interventions within scope
Strong skills in health education, harm reduction, and youth-focused disease prevention
Knowledge of trauma-informed, anti-oppressive, and culturally safe care, including 2SLGBTQ+ inclusion
Effective communicator with strong collaboration and interpersonal skills
Demonstrated leadership, mentorship, and clinical decision-making abilities
Committed to continuous learning, quality improvement, and ethical, evidence-based practice
Bilingualism (English/French) and/or proficiency in a second language is an asset
