Service Requests
Request for services can be made at any time, either by calling us directly at 613-966-3100 or toll-free at 1-844-462-2647 (1-844-GO TO CMHS) or by filling out the online referral form below.

We work with local agencies for interpretation, translation, and cultural brokering. We can work alongside these services and will ensure they are in place for your appointment(s). If you require information presented in an accessible format or other communication supports, we can make those available upon your request.

You will be responded to within the next business day of your initial contact to gather some information and arrange an intake assessment. Referrals are not reviewed after hours or on weekends. Office hours are Monday to Friday, from 8:30 am to 4:30 pm.

After-hours crisis consultation is available after 4:30 pm and on weekends. Please contact us at 613-966-3100 or toll-free at 1-844-462-2647 (1-844-GO TO CMHS) and proceed by pressing option 2 to go to the answering service, who will then have our on-call worker respond to you.
