Service Requests

PLEASE NOTE THIS IS NOT AN EMERGENCY SERVICE. FOR EMERGENCIES, CALL 911 OR GO TO THE NEAREST HOSPITAL EMERGENCY DEPARTMENT.  

Request for services can be made at any time, either by phone or filling out our online referral form. We work with local agencies for interpretation, translation, and cultural brokering. We can work alongside these services and will ensure they are in place for your appointment(s). If you require information presented in an accessible format or other communication supports, we can make those available upon your request.

You will be responded to within five business days of your web referral to gather some information and arrange an intake assessment. If you would like to speak to someone sooner to complete your intake, please phone us at 613-966-3100 ext. 2. Referrals are not reviewed after hours or on weekends. If you would like to make a referral but do not live within our service area, please contact us directly at 613-966-3100 to discuss your referral.

Some rural and northern addresses don’t always show up correctly in our system. If your address isn’t recognized or appears ineligible, it may just be a mapping issue, not a reflection of actual service availability. If this happens, please call us directly and we’ll be happy to confirm your eligibility and help you get connected.

Friday: 8:30AM – 4:30PM
